We achieve this with a designated team assigned to your discharged patient and facility. Thus, we establish a relationship between you and us, in order to follow your patient. Our goal is to work with your patient to ensure that the discharge plan is followed faithfully
Our engagement begins with a call from out LVN provider within 24 hours of discharge. During this call the LVN will review our program and answer any questions about our services. The next call will occur within 24-48 hours of the LVN call. The nurse practitioner will review the discharge plan with the patient, emphasizing compliance with your discharge plan and answer any questions.
The nurse practitioner will again call the patient within 7 and 14 days after discharge from the hospital to address any issues that may have arisen. Also, the nurse practitioner will contact you (Primary Care Physician), if any concerns or care issues arise. The ONclick support physician will have no direct contact with the patient.
After 30 days past hospitalization your patient will be discharged from the service.
