Our transition team is passionate and dedicated to creating a “Culture of Service Excellence” to ensure that you have a successful transition back to normal life.
We understand that navigating your discharge plan from home and be complex and transition team will be there step by step to support you through it.
Within a few hours after your discharge you will receive a text from us alerting you to expect a call from us. A Clinical member of our care team will first and foremost, make sure you have made a safe transition home. Thereafter, they will review your discharge plan with you or your caregiver and answer any questions about medications or other concerns.
During the call they will also schedule a call to have a Nurse Practitioner to call you within 24-48 hours to review your medications and potential resources available to you. They will also schedule an additional call with you within the following 7-14 days to check back to make sure your successful following your discharge plans.
Although we never take the place of your Primary Care Physician, we will keep them updated with anything we discover while we are supporting you.
After 30 days post hospitalization you will be discharged from the service.
